What is Novox Carprofen - Generic to Rimadyl?

Novox is a generic equivalent for Rimadyl (carprofen). It's a non-steroidal anti-inflammatory (NSAID) used to treat pain and inflammation due to arthritis and other joint diseases, including hip dysplasia. It's also used to reduce fever and to help control postoperative pain associated with soft tissue and orthopedic surgery in dogs. Novox is available as caplets (non-chewable) and liver-flavored chewable tablets. Both are scored for easy dosing and administration. Novox requires a prescription from your veterinarian.

How does Novox Carprofen work?

Novox works by reducing hormones that cause pain and inflammation in your dog's body.

Talk to your veterinarian or pharmacist before giving any other prescription or over-the-counter medications to your pet. Should not be given to pregnant or lactating dogs, or dogs used for breeding. Novox Carprofen should not be given to humans or cats. Seek medical assistance if cats or humans accidentally take Novox Carprofen. Your dog should not be given Novox Carprofen if he/she has had an allergic reaction to carprofen, the active ingredient of Novox. Do not give your dog Novox Carprofen if he/she has had an allergic reaction to aspirin or other NSAIDs (for example deracoxib, etodolac, firocoxib, meloxicam, phenylbutazone or tepoxalin) such as hives, facial swelling, or red or itchy skin.

What is the most important information I should know about Novox Carprofen?

Novox is a prescription medication that is FDA approved for use in dogs. Novox is available as 25 mg, 75 mg and 100 mg capsule shaped scored non-chewable caplets, and as 25 mg, 75 mg and 100 mg liver-flavored scored chewable tablets. The usual total daily dose in dogs is 2 mg per pound given as a single daily dose or divided and administered as 1 mg per pound twice a day. Contact the veterinarian if your pet has bloody, black or tarry stools as these symptoms could indicate damage to the stomach or intestines, which could be dangerous. As with all prescribed medicines, Novox Carprofen should only be given to the dog for which it was prescribed. It should be given to your dog only for the condition for which it was prescribed. It is important to periodically discuss your dog's response to Novox Carprofen at regular check ups. Your veterinarian will best determine if your dog is responding as expected and if your dog should continue receiving Novox Carprofen. Before giving your pet any prescription or over the counter medications, check with your veterinarian or pharmacist.

What should I discuss with my veterinarian before giving Novox Carprofen to my pet?

Tell your veterinarian if your pet is pregnant or lactating. Tell your veterinarian if your pet is being given any other medications, including those you can get over the counter. Also, tell your veterinarian if your dog has any other medical problems or allergies. Talk to your veterinarian before giving your pet any new prescription or over the counter medicines. Tell your veterinarian if your dog has ever experienced side effects from Novox or other NSAIDs such as aspirin, had digestive upset (vomiting and/or diarrhea), liver disease, kidney disease, or a bleeding disorder (for example, Von Willebrand's disease).

How should Novox Carprofen be given?

Give this medication exactly as directed by your veterinarian. If you do not understand the directions ask the pharmacist or veterinarian to explain them to you. Novox should be given with food. Keep plenty of water available for your pet. Store Novox Carprofen Caplets at room temperature, away from moisture and heat. Store 25 mg and 75 mg Novox Carprofen Chewable Tablets at 59-86F (15-30C). Store 100 mg Novox Carprofen Chewable Tablets at controlled room temperature, 68-77F (20-25C). Use half-tablets within 30 days. Keep this medication away from children and pets. Do not give Novox Carprofen to cats. Novox Carprofen Chewable Tablets are formulated to taste good to pets. Keep Novox Carprofen in a secured storage area out of the reach of your dog and other pets. If your dog ingests more than your veterinarian prescribed, or if your other pets accidentally take Novox Carprofen, contact your veterinarian right away.

What are the possible side effects of Novox Carprofen?

Adverse reactions may include decreased appetite, vomiting, diarrhea, dark or tarry stools, increased water consumption, increased urination, pale gums due to anemia, yellowing of gums, skin or white of the eye due to jaundice, lethargy, incoordination, seizure, or behavioral changes. Serious adverse reactions associated with this drug class can occur without warning and in rare situations result in death. Pet parents should be advised to discontinue Novox Carprofen therapy and contact their veterinarian immediately if signs of intolerance are observed. The vast majority of patients with drug-related adverse reactions have recovered when the signs are recognized, the drug is withdrawn, and veterinary care, if appropriate, is initiated. Talk to your veterinarian about any side effect that seems unusual or bothersome to your dog.

What happens if I miss giving a dose of Novox?

Give the missed dose as soon as you remember. However, if is almost time for the next regularly scheduled dose, skip the missed dose and give the next one as directed. Do not give a double dose of the medication.

What should I avoid while giving Novox Carprofen to my pet?

Many over the counter cough, cold, allergy and pain medicines contain aspirin or other NSAIDs similar to Novox Carprofen. Before giving any prescription or over the counter medications talk to the veterinarian or pharmacist.

What other drugs will affect Novox Carprofen?

Tell your veterinarian if your pet is being given aspirin; another NSAID such as etodolac, carprofen or piroxicam; an over the counter cough, cold, allergy or pain medication; warfarin; furosemide; a steroid such as prednisone, insulin; or probenecid. Novox Carprofen should not be given with other NSAIDs (for example, aspirin, deracoxib, etodolac, firocoxib, meloxicam, tepoxalin) or steroids (for example, cortisone, dexamethasone, prednisone, triamcinolone). Drugs other than those listed may also interact with Novox Carprofen. Talk to your veterinarian before giving your pet any prescription or over the counter medicines.

Novox Carprofen - Generic to Rimadyl Directions:

  • The recommended dosage for oral administration to dogs is 2 mg/lb of body weight daily. However, Novox Carprofen should be given only as prescribed by your veterinarian.
  • Use the lowest effective dose for the shortest duration consistent with individual response.
  • Novox Carprofen can be given as a single full dose once a day or divided in half and given twice a day.
  • Novox Carprofen is for dogs only. Do not give to cats.
  • Novox Carprofen Caplets and Chewable Tablets are scored and dosage should be calculated in half-tablet increments. Tablets can be halved by placing the tablet on a hard surface and pressing down on both sides of the score.

Over-the-Counter Nonsteroidal Anti-inflammatory Drugs

NSAIDs are the most commonly used class of human medications in the world. Because of their widespread availability and use, acute accidental ingestion of human NSAIDs in dogs and cats is quite common. Ibuprofen , aspirin , and naproxen are some of the most commonly encountered NSAIDs in pet animals.

NSAIDs inhibit the enzyme cyclooxygenase (COX; also referred to as prostaglandin synthetase), blocking the production of prostaglandins. It is believed that most NSAIDs act through COX inhibition, although they may also have other mechanisms of action. (Also see Nonsteroidal Anti-inflammatory Drugs.)

Ibuprofen , 2-(4-isobutylphenyl) propionic acid, is used for its anti-inflammatory, antipyretic, and analgesic properties in animals and people. It is rapidly absorbed orally in dogs, with peak plasma concentrations seen in 30 min to 3 hr. Presence of food can delay absorption and the time to reach peak plasma concentration. The mean elimination half-life is

4.6 hr. Ibuprofen is metabolized in the liver to several metabolites, which are mainly excreted in the urine within 24 hr. The major metabolic pathway is via conjugation with glucuronic acid, sometimes preceded by oxidation and hydroxylation.

Ibuprofen has been recommended in dogs at 5 mg/kg. However, prolonged use at this dosage may cause gastric ulcers and perforations. GI irritation or ulceration, GI hemorrhage, and renal damage are the most commonly reported toxic effects of ibuprofen ingestion in dogs. In addition, CNS depression, hypotension, ataxia, cardiac effects, and seizures can be seen. Ibuprofen has a narrow margin of safety in dogs. Dogs dosed with ibuprofen at 8–16 mg/kg/day, PO for 30 days, showed gastric ulceration or erosions, along with other clinical signs of GI disturbances. An acute single ingestion of 100–125 mg/kg can lead to vomiting, diarrhea, nausea, abdominal pain, and anorexia. Renal failure may follow dosages of 175–300 mg/kg. CNS effects (ie, seizures, ataxia, depression, coma) in addition to renal and GI signs can be seen at dosages >400 mg/kg. Dosages >600 mg/kg are potentially lethal in dogs.

Cats are susceptible to ibuprofen toxicosis at approximately half the dosage required to cause toxicosis in dogs. Cats are especially sensitive, because they have limited glucuronyl-conjugating capacity. Ibuprofen toxicity is more severe in ferrets than in dogs that consume similar dosages. Typical toxic effects of ibuprofen in ferrets involve the CNS, GI, and renal systems.

Aspirin (acetylsalicylic acid), the salicylate ester of acetic acid, is the prototype of salicylate drugs. It is a weak acid derived from phenol. The oral bioavailability of aspirin may vary because of differences in drug formulation. Aspirin reduces prostaglandin and thromboxane synthesis by COX inhibition. Salicylates also uncouple mitochondrial oxidative phosphorylation and inhibit specific dehydrogenases. Platelets are incapable of synthesizing new cyclooxgenase, leading to an effect on platelet aggregation.

Aspirin is rapidly absorbed from the stomach and proximal small intestine in monogastric animals. The rate of absorption depends on gastric emptying, tablet disintegration rates, and gastric pH. Peak salicylate levels are reached 0.5–3 hr after ingestion. Topically applied salicylic acid can be absorbed systemically.

Aspirin is hydrolyzed to salicylic acid by esterases in the liver and, to a lesser extent, in the GI mucosa, plasma, RBCs, and synovial fluid. Salicylic acid is 50%–70% protein bound, especially to albumin. Salicylic acid readily distributes to extracellular fluids and to the kidneys, liver, lungs, and heart. Salicylic acid is eliminated by hepatic conjugation with glucuronide and glycine. Renal clearance is enhanced by an alkaline urinary pH. There are significant differences in the elimination and biotransformation of salicylates among different species. Plasma half-lives vary from 1–37.6 hr in animals.

Aspirin toxicosis is usually characterized by depression, fever, hyperpnea, seizures, respiratory alkalosis, metabolic acidosis, coma, gastric irritation or ulceration, liver necrosis, or increased bleeding time. Seizures may occur as a consequence of severe intoxication, although the exact cause is unknown.

Cats are deficient in glucuronyl transferase and have a prolonged excretion of aspirin (half-life 37.5 hr). No clinical signs of toxicosis occurred when cats were given aspirin at 25 mg/kg every 48 hr for up to 4 wk. Dosages of 5 grains (325 mg), bid, can be lethal to cats.

Dogs tolerate aspirin better than cats; however, prolonged use can lead to development of gastric ulcers. Regular aspirin at dosages of 25 mg/kg, tid, has caused mucosal erosions in 50% of dogs after 2 days. Gastric ulcers were seen by day 30 in 66% of dogs given aspirin at 35 mg/kg, PO, tid. Similarly, 43% of dogs given aspirin at 50 mg/kg, PO, bid, showed gastric ulcers after 5–6 wk of dosing. Acute ingestion of 450–500 mg/kg can cause GI disturbances, hyperthermia, panting, seizures, or coma. Alkalosis due to stimulation of the respiratory center can occur early in the course of intoxication. Metabolic acidosis with an increased anion gap usually develops later.

Naproxen , a propionic acid–derivative NSAID, is available OTC as an acid or the sodium salt. It is available as tablets or gel caps (200–550 mg) or as a suspension (125 mg/5 mL). Structurally and pharmacologically, naproxen is similar to carprofen and ibuprofen . In people and dogs, it is used for its anti-inflammatory, analgesic, and antipyretic properties.

Oral absorption of naproxen in dogs is rapid, with peak plasma concentration reached in 0.5–3 hr. The reported elimination half-life in dogs is 34–72 hr. Naproxen is highly protein bound (>99%). In dogs, naproxen is primarily eliminated through the bile, whereas in other species, the primary route of elimination is through the kidneys. The long half-life of naproxen in dogs appears to be because of its extensive enterohepatic recirculation.

Several cases of naproxen toxicosis in dogs have been reported. Dosages of 5.6–11.1 mg/kg, PO, for 3–7 days have caused melena, frequent vomiting, abdominal pain, perforating duodenal ulcer, weakness, stumbling, pale mucous membranes, regenerative anemia, neutrophilia with a left shift, increased BUN and creatinine, and decreased total protein. Acute toxicity from a single oral dose of 35 mg/kg has been reported. Cats may be more sensitive to naproxen toxicity than dogs because of their limited glucuronyl-conjugating capacity.

Treatment of NSAID toxicosis consists of early decontamination, protection of the GI tract and kidneys, and supportive care. Vomiting should be induced in recent exposures, followed by administration of activated charcoal with a cathartic. Activated charcoal can be repeated in 6–8 hr to prevent NSAID reabsorption from enterohepatic recirculation. Use of H2-receptor antagonists ( ranitidine , famotidine , cimetidine ) may not prevent GI ulcers but can be useful in treating them. Omeprazole , a proton-pump inhibitor used to inhibit gastric acid secretions, can be used instead of an H2 blocker at 0.5–1 mg/kg/day, PO, in dogs. Sucralfate (dogs: 0.5–1 g, PO, bid-tid; cats: 0.25–0.5 tablet, PO, bid-tid) reacts with hydrochloric acid in the stomach and forms a paste-like complex that binds to the proteins in ulcers and protects them from further damage. Because sucralfate requires an acidic environment, it should be given ≥30 min before administering H2 antagonists. Misoprostol (dogs: 1–3 mcg/kg, PO, tid) has been shown to prevent GI ulceration when used concomitantly with aspirin and other NSAIDs.

IV fluids should be given at a diuretic rate if the potential for renal damage exists. Alkalinization of the urine with sodium bicarbonate results in ion trapping of salicylates in kidney tubules and can increase their excretion. However, ion trapping should be used judiciously and only in cases when the acid-base balance can be monitored closely. Baseline renal function should be monitored and rechecked at 24, 48, and 72 hr. Prognosis depends on the dose ingested and how soon the animal receives treatment after exposure.

Acetaminophen

Acetaminophen is a synthetic nonopiate derivative of p-aminophenol widely used in people for its antipyretic and analgesic properties. Its use has largely replaced salicylates because of the reduced risk of gastric ulceration.

Acetaminophen is rapidly absorbed from the GI tract. Peak plasma concentrations are usually seen within an hour but can be delayed with extended-release formulations. It is uniformly distributed into most body tissues. Protein binding varies from 5%–20%. The metabolism of acetaminophen involves two major conjugation pathways in most species. Both involve cytochrome P450 metabolism, followed by glucuronidation or sulfation.

Cats are more sensitive to acetaminophen toxicosis, because they are deficient in glucuronyl transferase and therefore have limited capacity to glucuronidate this drug. In cats, acetaminophen is primarily metabolized via sulfation; when this pathway is saturated, toxic metabolites are produced. In dogs, signs of acute toxicity are usually not seen unless the dosage of acetaminophen exceeds 100 mg/kg. Clinical signs of methemoglobinemia have been reported in 3 of 4 dogs at 200 mg/kg. Toxicity can be seen at lower dosages with repeated exposures. In cats, toxicity can occur with 10–40 mg/kg.

Methemoglobinemia and hepatotoxicity characterize acetaminophen toxicosis. Renal injury is also possible. Acute keratoconjunctivitis sicca has been reported in some dogs after acetaminophen ingestion. Cats primarily develop methemoglobinemia within a few hours, followed by Heinz body formation. Methemoglobinemia makes mucous membranes brown or muddy in color and is usually accompanied by tachycardia, hyperpnea, weakness, and lethargy. Other clinical signs of acetaminophen toxicity include depression, weakness, hyperventilation, icterus, vomiting, hypothermia, facial or paw edema, cyanosis, dyspnea, hepatic necrosis, and death. Liver necrosis is more common in dogs than in cats. Liver damage in dogs is usually seen 24–36 hr after ingestion. Centrilobular necrosis is the most common form of hepatic necrosis seen with acetaminophen toxicity.

The objectives of treating acetaminophen toxicosis are early decontamination, prevention or treatment of methemoglobinemia and hepatic damage, and provision of supportive care. A Schirmer tear test (to confirm keratoconjunctivitis) can be used if necessary. Induction of emesis is useful when performed early. This should be followed by administration of activated charcoal with a cathartic. Activated charcoal may be repeated, because acetaminophen undergoes some enterohepatic recirculation.

Administration of N- acetylcysteine (NAC), a sulfur-containing amino acid, can reduce the extent of liver injury or methemoglobinemia. NAC provides sulfhydryl groups, directly binds with acetaminophen metabolites to enhance their elimination, and serves as a glutathione precursor. It is available as a 10% or 20% solution. The loading dose is 140 mg/kg of a 5% solution IV or PO (diluted in 5% dextrose or sterile water), followed by 70 mg/kg, PO, qid for generally seven or more treatments (some authors recommend up to 17 doses). Vomiting can occur with oral NAC. NAC is not labeled for IV use; however, it can be administered as a slow IV (over 15–20 min) with a 0.2-micron bacteriostatic filter. Activated charcoal and oral NAC should be administered 2 hr apart, because activated charcoal could adsorb NAC.

Liver enzymes should be monitored and rechecked at 24 and 48 hr. The animal should also be monitored for methemoglobinemia, Heinz body anemia, and hemolysis. Fluids and blood transfusions should be given as needed. Ascorbic acid (30 mg/kg, PO or injectable, bid-qid) may further reduce methemoglobin levels. Cimetidine (5–10 mg/kg, PO, IM, or IV), a cytochrome P450 inhibitor, may help reduce formation of toxic metabolites and prevent liver damage in dogs only. Cimetidine should not be used in cats. In vitro evidence indicates that use of cimetidine in cats can produce more toxic metabolites of acetaminophen . S-Adenosyl methionine has been suggested as an adjunct to manage acute or chronic hepatic injury at 18 mg/kg, PO, for 1–3 mo in dogs and cats.

Giving paracetamol to dogs

My crazy greyhound ran into a tree stump today & took the top layer of his skin off (about 1.5" wound) He walked home okay & he is now a bit sore as you would expect. I am usually the type of person who goes to the vet's with every small worry/query but (apart from it being Bank Holiday) TBH at the moment I think this is a case of first aid. I have not bathed it as it looks quite clean & think this might irritate it. I have NEVER given my dogs human medecine before because I have read that it can be poison to dogs but I hate to see him in discomfort. My friend has recently been told by her dog's breeder to give 5 or 10ml of children's neurofen for growing pains in her puppy and I would like to ask if anyone has any experience of giving children's paracetemol or aspirin to dogs. At the moment my dog is fine but interested if you have any experience or knowledge of this. I do know he would need a very small dose of anything as being a grey has no body fat.

Quite a different situation, but one of my labs, shortly after having her hip replaced, got hold of a strip of ibuprofen tablets and ate the lot!

Don't give anything without checking with a vet.

Paracetamol can be fatal for cats. I think it is crazy to give it to a dog too.

Do NOT give paracetamol or aspirin.No No no.Many animals react completely differently to different drugs and they may well be totally toxic to dogs.(I think aspirin can kill a cat for instance)There is an anti inflamatory suitable for dogs that you need to get from your vet,as also,as you mention,doseage is also crucial.Call your vet in the morning,they will need to weigh your dog,take the breed type into consideration,and prescribe the appropriate drug.

In general human pain drugs are NOT suitable for dogs. I am horrified that a breeder is advising giving ibuprofen to puppies - this is likely to cause kidney failure. Apart from anything else there's no such diagnosis as "growing pains" and the puppy needs to be seen by a vet.

Well said, OhFuck. I am a vet as well. My advice is very simple: never give a dog (or cat) paracetamol or ibuprofen.

Another vet here - I'll go slightly against the grain here and say actually in the correct circumstances paracetamol is an option for dogs. However, it should never be given without the instruction of a veterinary surgeon as it has the potential to cause problems and dosing will not be the same as for a small child. There are far better and safer products manufactured for pain control in dogs and it would be a very rare event indeed where a vet in practice is likely to use it. It is never suitable for cats. As with the previous posters here though I will reiterate that ibuprofen is totally unsuitable for dogs or cats under any circumstances and I have seen first hand the effects of ibuprofen toxicity. It doesn't need much for a potentially life threatening problem.

Another vet (gosh there's a few of us!) and I agree with beautifulgirls. Paracetamol can be given to dogs, but phone your OOH number in future and they will at least be able to give you advice on a dosage without needing to necessarily see the dog.

phew ! it was a weekend and my dog had run into a tree ,luckily there was no sign of injury other than a bit of a limp and the vet said i could give him a small dose of calpol and then bring him in on monday morning ,didnt need to as he fully recovered ,the great numpty

We were advised to give our old lab aspirin if she were particularly stiff, but only on occasion. We did once or twice and then moved on to the proper dog pain relief which I can't remember the name of now!

bedlambeast yup, because its actually better to let it all out your system and treat with natural yoghurt/koalin/balnd diet etc (that goes for us too) but it is foul. And totally not licensed. Oh well

I have dogs all my life. Aspirin and paracetamol are absolutely fine in small doses. The only reason all you vets on here are saying we should use licensed medication is due to the fact you can charge £40 for something that costs 19p in Asdas. same old system full of money grabbing scumbags!

Cammeag yes they are okay in small doses under veterinary advice for dogs. We are not saying this to scam you we are saying this to prevent potential serious life threatening problems. Actually if you wish to argue the licensed route then please write to the veterinary medicines directorate who legally force us to prescribe the licensed drugs rather than cheaper generics.

Hi My Friends vet and my own dog's vet have both confirmed it is OK to give children's Priiton to dogs in emergency situations. I hope this is correct, any other vets in reading may comment, but I did administer when my dog was stung by wasps and there was no way of getting vet treatment quickly, he did not suffer any side effects and the wasp stings subsided quickly after taking the medication. I have also given to my Staffie when she developed an itchy grass rash, it worked for her.

Piriton is fine but talk to your vet about dosage. It can cause sedation in some dogs.

I should also add that it's important to seek veterinary attention if your pet is stung in the mouth. There is a small risk of swelling compromising their airway, which would need more aggressive treatment than simply Piriton.

Whoaaa don't ever give human tablets to a dog without the vets advice. Aspirin can just burn a hole in a dogs stomach, and giving neurofen to puppies is frankly irresponsible. Go to www.vetmedsdirect.co.uk for advice or see your vet

Fludrocortisone Acetate Tablets USP

Description

Florinef Acetate (Fludrocortisone Acetate Tablets USP) contains fludrocortisone acetate, a synthetic adrenocortical steroid possessing very potent mineralocorticoid properties and high glucocorticoid activity; it is used only for its mineralocorticoid effects. The chemical name for fludrocortisone acetate is 9-fluoro- 11ß,17,21-trihydroxypregn-4-ene-3,20-dione 21-acetate; its graphic formula is:

Florinef Acetate is available for oral administration as scored tablets providing 0.1 mg fludrocortisone acetate per tablet. Inactive ingredients: calcium phosphate, corn starch, lactose, magnesium stearate, sodium benzoate, and talc.

Clinical Pharmacology

Corticosteroids are thought to act, at least in part, by controlling the rate of synthesis of proteins. Although there are a number of instances in which the synthesis of specific proteins is known to be induced by corticosteroids, the links between the initial actions of the hormones and the final metabolic effects have not been completely elucidated.

The physiologic action of fludrocortisone acetate is similar to that of hydrocortisone. However, the effects of fludrocortisone acetate, particularly on electrolyte balance, but also on carbohydrate metabolism, are considerably heightened and prolonged. Mineralocorticoids act on the distal tubules of the kidney to enhance the reabsorption of sodium ions from the tubular fluid into the plasma; they increase the urinary excretion of both potassium and hydrogen ions. The consequence of these three primary effects together with similar actions on cation transport in other tissues appear to account for the entire spectrum of physiological activities that are characteristic of mineralocorticoids. In small oral doses, fludrocortisone acetate produces marked sodium retention and increased urinary potassium excretion. It also causes a rise in blood pressure, apparently because of these effects on electrolyte levels.

In larger doses, fludrocortisone acetate inhibits endogenous adrenal cortical secretion, thymic activity, and pituitary corticotropin excretion; promotes the deposition of liver glycogen; and, unless protein intake is adequate, induces negative nitrogen balance.

The approximate plasma half-life of fludrocortisone (fluorohydrocortisone) is 3.5 hours or more and the biological half-life is 18 to 36 hours.

Indications and Usage

Florinef Acetate is indicated as partial replacement therapy for primary and secondary adrenocortical insufficiency in Addison’s disease and for the treatment of salt-losing adrenogenital syndrome.

Contraindications

Corticosteroids are contraindicated in patients with systemic fungal infections and in those with a history of possible or known hypersensitivity to these agents.

BECAUSE OF ITS MARKED EFFECT ON SODIUM RETENTION, THE USE OF FLUDROCORTISONE ACETATE IN THE TREATMENT OF CONDITIONS OTHER THAN THOSE INDICATED HEREIN IS NOT ADVISED .

Corticosteroids may mask some signs of infection, and new infections may appear during their use. There may be decreased resistance and inability to localize infection when corticosteroids are used. If an infection occurs during fludrocortisone acetate therapy, it should be promptly controlled by suitable antimicrobial therapy.

Prolonged use of corticosteroids may produce posterior subcapsular cataracts, glaucoma with possible damage to the optic nerves, and may enhance the establishment of secondary ocular infections due to fungi or viruses.

Average and large doses of hydrocortisone or cortisone can cause elevation of blood pressure, salt and water retention, and increased excretion of potassium. These effects are less likely to occur with the synthetic derivatives except when used in large doses. However, since fludrocortisone acetate is a potent mineralocorticoid, both the dosage and salt intake should be carefully monitored in order to avoid the development of hypertension, edema, or weight gain. Periodic checking of serum electrolyte levels is advisable during prolonged therapy; dietary salt restriction and potassium supplementation may be necessary. All corticosteroids increase calcium excretion.

Patients should not be vaccinated against smallpox while on corticosteroid therapy. Other immunization procedures should not be undertaken in patients who are on corticosteroids, especially on high dose, because of possible hazards of neurological complications and a lack of antibody response.

The use of Florinef Acetate (Fludrocortisone Acetate Tablets USP) in patients with active tuberculosis should be restricted to those cases of fulminating or disseminated tuberculosis in which the corticosteroid is used for the management of the disease in conjunction with an appropriate antituberculous regimen. If corticosteroids are indicated in patients with latent tuberculosis or tuberculin reactivity, close observation is necessary since reactivation of the disease may occur. During prolonged corticosteroid therapy these patients should receive chemoprophylaxis.

Children who are on immunosuppressant drugs are more susceptible to infections than healthy children. Chicken pox and measles, for example, can have a more serious or even fatal course in children on immunosuppressant corticosteroids. In such children, or in adults who have not had these diseases, particular care should be taken to avoid exposure. If exposed, therapy with varicella zoster immune globulin (VZIG) or pooled intravenous immunoglobulin (IVIG), as appropriate, may be indicated. If chicken pox develops, treatment with antiviral agents may be considered.

Precautions

Adverse reactions to corticosteroids may be produced by too rapid withdrawal or by continued use of large doses.

To avoid drug-induced adrenal insufficiency, supportive dosage may be required in times of stress (such as trauma, surgery, or severe illness) both during treatment with fludrocortisone acetate and for a year afterwards.

There is an enhanced corticosteroid effect in patients with hypothyroidism and in those with cirrhosis.

Corticosteroids should be used cautiously in patients with ocular herpes simplex because of possible corneal perforation.

The lowest possible dose of corticosteroid should be used to control the condition being treated. A gradual reduction in dosage should be made when possible.

Psychic derangements may appear when corticosteroids areused. These may range from euphoria, insomnia, mood swings, personality changes, and severe depression to frank psychotic manifestations. Existing emotional instability or psychotic tendencies may also be aggravated by corticosteroids.

Aspirin should be used cautiously in conjunction with corticosteroids in patients with hypoprothrombinemia.

Corticosteroids should be used with caution in patients with nonspecific ulcerative colitis if there is a probability of impending perforation, abscess, or other pyogenic infection. Corticosteroids should also be used cautiously in patients with diverticulitis, fresh intestinal anastomoses, active or latent peptic ulcer, renal insufficiency, hypertension, osteoporosis, and myasthenia gravis.

Information for Patients

The physician should advise the patient to report any medical history of heart disease, high blood pressure, or kidney or liver disease and to report current use of any medicines to determine if these medicines might interact adversely with fludrocortisone acetate (see Drug Interactions ).

Patients who are on immunosuppressant doses of corticosteroids should be warned to avoid exposure to chicken pox or measles and, if exposed, to obtain medical advice.

The patient’s understanding of his steroid-dependent status and increased dosage requirement under widely variable conditions of stress is vital. Advise the patient to carry medical identification indicating his dependence on steroid medication and, if necessary, instruct him to carry an adequate supply of medication for use in emergencies.

Stress to the patient the importance of regular follow-up visits to check his progress and the need to promptly notify the physician of dizziness, severe or continuing headaches, swelling of feet or lower legs, or unusual weight gain.

Advise the patient to use the medicine only as directed, to take a missed dose as soon as possible, unless it is almost time for the next dose, and not to double the next dose.

Inform the patient to keep this medication and all drugs out of the reach of children.

Laboratory Tests

Patients should be monitored regularly for blood pressure determinations and serum electrolyte determinations (see WARNINGS ).

Drug Interactions

When administered concurrently, the following drugs may interact with adrenal corticosteroids.

Amphotericin B or potassium-depleting diuretics (benzothiadiazines and related drugs, ethacrynic acid and furosemide)—enhanced hypokalemia. Check serum potassium levels at frequent intervals; use potassium supplements if necessary (see WARNINGS ).

Digitalis glycosides —enhanced possibility of arrhythmias or digitalis toxicity associated with hypokalemia. Monitor serum potassium levels; use potassium supplements if necessary.

Oral anticoagulants —decreased prothrombin time response. Monitor prothrombin levels and adjust anticoagulant dosage accordingly.

Antidiabetic drugs (oral agents and insulin)—diminished antidiabetic effect. Monitor for symptoms of hyperglycemia; adjust dosage of antidiabetic drug upward if necessary.

Aspirin —increased ulcerogenic effect; decreased pharmacologic effect of aspirin. Rarely salicylate toxicity may occur in patients who discontinue steroids after concurrent high-dose aspirin therapy. Monitor salicylate levels or the therapeutic effect for which aspirin is given; adjust salicylate dosage accordingly if effect is altered (see PRECAUTIONS , General ).

Barbiturates, phenytoin, or rifampin —increased metabolic clearance of fludrocortisone acetate because of the induction of hepatic enzymes. Observe the patient for possible diminished effect of steroid andincrease the steroid dosage accordingly.

Anabolic steroids (particularly C-17 alkylated androgens such as oxymetholone, methandrostenolone, norethandrolone, and similar compounds)—enhanced tendency toward edema. Use caution when giving these drugs together, especially in patients with hepatic or cardiac disease.

Vaccines —neurological complications and lack of antibody response (see WARNINGS ).

Estrogen —increased levels of corticosteroid-binding globulin, thereby increasing the bound (inactive) fraction; this effect is at least balanced by decreased metabolism of corticosteroids. When estrogen therapy is initiated, a reduction in corticosteroid dosage may be required, and increased amounts may be required when estrogen is terminated.

Drug/Laboratory Test Interactions

Corticosteroids may affect the nitrobluetetrazolium test for bacterial infection and produce false-negative results.

Carcinogenesis, Mutagenesis, Impairment of Fertility

Adequate studies have not been performed in animals to determine whether fludrocortisone acetate has carcinogenic or mutagenic activity or whether it affects fertility in males or females.

Adequate animal reproduction studies have not been conducted with fludrocortisone acetate. However, many corticosteroids have been shown to be teratogenic in laboratory animals at low doses. Teratogenicity of these agents in man has not been demonstrated. It is not known whether fludrocortisone acetate can cause fetal harm when administered to a pregnant woman or can affect reproduction capacity. Fludrocortisone acetate should be given to a pregnant woman only if clearly needed.

Infants born of mothers who have received substantial doses of fludrocortisone acetate during pregnancy should be carefully observed for signs of hypoadrenalism.

Maternal treatment with corticosteroids should be carefully documented in the infant’s medical records to assist in follow up.

Nursing Mothers

Corticosteroids are found in the breast milk of lactating women receiving systemic therapy with these agents. Caution should be exercised when fludrocortisone acetate is administered to a nursing woman.

Pediatric Use

Safety and effectiveness in children have not been established.

Growth and development of infants and children on prolonged corticosteroid therapy should be carefully observed.

Geriatric Use

Elderly subjects may commonly have conditions that may be exacerbated by fludrocortisone therapy including, but not limited to, hypertension, edema, hypokalemia, congestive heart failure, cataracts, glaucoma, increased intraocular pressure, renal insufficiency, and osteoporisis (see WARNINGS , PRECAUTIONS and ADVERSE REACTIONS ). Elderly subjects may also commonly be taking concomitant drug therapy such as digitalis glycosides, oral anticoagulants, antidiabetic drugs (oral agents and insulin), and aspirin which may interact with fludrocortisone (see PRECAUTIONS - Drug Interactions ).

In general, dose selection for an elderly patient should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal, or cardiac function, and of concomitant disease or other drug therapy.

Adverse Reactions

Most adverse reactions are caused by the drug’s mineralocorticoid activity (retention of sodium and water) and include hypertension, edema, cardiac enlargement, congestive heart failure, potassium loss, and hypokalemic alkalosis.

When fludrocortisone is used in the small dosages recommended, the glucocorticoid side effects often seen with cortisone and its derivatives are not usually a problem; however the following untoward effects should be kept in mind, particularly when fludrocortisone is used over a prolonged period of time or in conjunction with cortisone or a similar glucocorticoid.

Musculoskeletal —muscle weakness, steroid myopathy, loss of muscle mass, osteoporosis, vertebral compression fractures, aseptic necrosis of femoral and humeral heads, pathologic fracture of long bones, and spontaneous fractures.

Gastrointestinal —peptic ulcer with possible perforation and hemorrhage, pancreatitis, abdominal distention, and ulcerative esophagitis.

Dermatologic —impaired wound healing, thin fragile skin, bruising, petechiae and ecchymoses, facial erythema, increased sweating, subcutaneous fat atrophy, purpura, striae, hyperpigmentation of the skin and nails, hirsutism, acneiform eruptions, and hives; reactions to skin tests may be suppressed.

Neurological —convulsions, increased intracranial pressure with papilledema (pseudotumor cerebri) usually after treatment, vertigo, headache, and severe mental disturbances.

Endocrine —menstrual irregularities; development of the cushingoid state; suppression of growth in children; secondary adrenocortical and pituitary unresponsiveness, particularly in times of stress (e.g., trauma, surgery, or illness); decreased carbohydrate tolerance; manifestations of latent diabetes mellitus; and increased requirements for insulin or oral hypoglycemic agents in diabetics.

Ophthalmic —posterior subcapsular cataracts, increased intraocular pressure, glaucoma, and exophthalmos.

Metabolic —hyperglycemia, glycosuria, and negative nitrogen balance due to protein catabolism.

Allergic Reactions —allergic skin rash, maculopapular rash, and urticaria.

Other adverse reactions that may occur following the administration of a corticosteroid are necrotizing angiitis, thrombophlebitis, aggravation or masking of infections, insomnia, syncopal episodes, and anaphylactoid reactions.

Overdosage

Development of hypertension, edema, hypokalemia, excessive increase in weight, and increase in heart size are signs of overdosage of fludrocortisone acetate. When these are noted, administration of the drug should be discontinued, after which the symptoms will usually subside within several days; subsequent treatment with fludrocortisone acetate should be with a reduced dose. Muscular weakness may develop due to excessive potassium loss and can be treated by administering a potassium supplement. Regular monitoring of blood pressure and serum electrolytes can help to prevent overdosage (see WARNINGS ).

Dosage and Administration

Dosage depends on the severity of the disease and the response of the patient. Patients should be continually monitored for signs that indicate dosage adjustment is necessary, such as remissions or exacerbations of the disease and stress (surgery, infection, trauma) (see WARNINGS and PRECAUTIONS , General ).

Addison’s Disease

In Addison’s disease, the combination of Florinef Acetate (Fludrocortisone Acetate Tablets USP) with a glucocorticoid such as hydrocortisone or cortisone provides substitution therapy approximating normal adrenal activity with minimal risks of unwanted effects.

The usual dose is 0.1 mg of Florinef Acetate daily, although dosage ranging from 0.1 mg three times a week to 0.2 mg daily has been employed. In the event transient hypertension develops as a consequence of therapy, the dose should be reduced to 0.05 mg daily. Florinef Acetate is preferably administered in conjunction with cortisone (10 mg to 37.5 mg daily in divided doses) or hydrocortisone (10 mg to 30 mg daily in divided doses).

Salt-Losing Adrenogenital Syndrome

The recommended dosage for treating the salt-losing adrenogenital syndrome is 0.1 mg to 0.2 mg of Florinef Acetate daily.

Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID), a class of drug that is commonly used for the relief of pain and inflammation associated with arthritis, headaches, menstrual cramps, and for the relief of mild fevers. Though ibuprofen is used commonly by people, dogs and cats are very sensitive to this drug and its use is not recommended. Dog-specific NSAIDs include common brands such as Rimadyl, Dermaxx, Previcox and Metacam. Currently, there are no NSAIDs designed for long-term use in cats. This class of drug is used sparingly in cats because it is poorly tolerated (it can result in severe kidney failure with overdose or repeated doses).

Pet owners should never give any medication to their dog or cat without consultation with their veterinarian. Some pet owners give NSAIDs, like ibuprofen, without realizing the dangers to their dog or cat. When pets ingest small doses of ibuprofen, it may result in severe stomach ulcers causing signs of vomiting, bloody vomitus, diarrhea, black-tarry stool, weakness, pale gums (a sign of anemia), abdominal pain, lethargy, and loss of appetite. With larger ingestions, kidney failure, liver failure and neurological problems (e.g., tremors, seizures) can develop.

I have an 8 yr old miniature wired dachshund that weighs three pounds. She recently was diagnosed with knee displasia and was put on prednisone. She is given 5mg twice a day & I was told I could give her aspirin as well. During my research, I have seen articles stating not to mix these too drugs. She is extremely small & I need to be very careful! Any advice would be greatly appreciated!

Thank you for being concerned enough about your dog's health to research this matter! You may have saved your dog's life. Mixing a steroid like prednisone with a non-steroidal anti-inflammatory (NSAID's) drug such as Rimadyl or aspirin can cause severe problems. NSAID's can have a side effect of causing stomach ulcers. This side effect is greatly increased when mixed with a steroid drug like prednisone. When these two drugs are together it can lead to a penetrating or bleeding ulcer in the stomach. With aspirin this can pose an even greater problem as aspirin can decrease the blood's ability to clot. You should not give an NSAID like aspirin to your dog until she has been off of steroids for at least two weeks. These drugs should definitely not be given simultaneously. If the prednisone is not enough pain control for your dog, you should talk to your veterinarian about another pain medication like tramadol, which would be safe to administer with prednisone.

Rimadyl recovery time for dogs?

My 5 1/2 yr old Lab was on Rimadyl for 14 days. His last dose was 7 days ago. In the past 7 days, his appetite has decreased and has been non-existent for the past 3 days. He has eaten nearly nothing in 3 days and I have tried literally everything in the house . He is also extremely lethargic and drowsy. He only gets up to go outside and drink water and he drinks a lot of it lately. Assuming the side effects are associated with Rimadyl, how long will it take him to recover from the drugs?

I have an 82 lb Chocolate lab, who two days ago, we took to the vet for some kind of infection on his toe. I have a 1 year old golden Retriever who was being treated for Demodex with high dose Ivermectin. A few days ago she developed Ivermectin Toxicity after being given her monthly dose of Comfortis. (The Vet said this was safe to do) Needless to say, it will be a while before she is fully recovered so what do we do about the mange and the monthly flea treatment and the heartworm prevention? She did well in the past with Revolution, but I am a bit concerned. Her neuro toxic symptoms were severe.
